Prescription REFILL/Request Form

In an effort to make filling your pet's prescriptions easier for you and us, you can now request a refill for your pet by submitting the following form. The prescription request must be approved by a doctor before we can fill it.

It takes us 12-24 hours to process refill requests. We will notify you via email or phone when your pet's prescription is approved and ready to be picked up.

After being filled, prescriptions can only be held at the front desk for 7 days before they are returned to our stock.
